Bulk Download (Download All) function leads to error message when the assessment name contains a #.

For example, if the assessment name is "Assessment #2", when user clicks on "Download All" link on the question score page, following error will be seen:

You do not have permission to access this media. If you are not currently logged in, please log in and try again. If you are logged in and believe you should be able to view this file, please contact your local support for help.

How to reproduce:
1. Create an assessment with File Upload type.
2. Published it.
3. Login as a student. Upload any file and submit the assessment.
4. Go to Question Score page. Click on the "Download All" link.
